CLINICAL SERVICES

Dr. Greenstein’s practice focuses exclusively on adults over 50 years of age with one or more the following concerns:

  • New onset depression, anxiety or other mental health struggles 

  • Chronic mental illness (depression, anxiety, bipolar disorder, schizophrenia, PTSD, etc.) 

  • Mental illness in the setting of medical comorbidity 

  • Concerns about memory or thinking or recent dementia diagnosis

  • Dementia related behaviors

  • Changes to behavior or personality

  • Polypharmacy

  • Substance use disorders

Dr. Greenstein provides the following clinical services:

  • Treatment of new onset and chronic depression, anxiety, PTSD and other mental illness in people experiencing age-related changes or medical illness 

  • Assessment of concerns regarding memory, cognition, thinking or changes in behavior

  • Diagnosis of neurocognitive disorders and the treatment of related behavioral symptoms

  • Medication management for alcoholism and other substance use disorders

  • Reduction of polypharmacy and simplification of complex medication regimens 

  • Family/caregiver education about neurocognitive disorders

  • Family/caregiver education about late life mental illness

All of Dr. Greenstein’s new patients undergo a comprehensive evaluation.

Here is what you can expect at your new patient evaluation: 

  • The initial visit typically takes place in-person and lasts for approximately two-to-three hours.

  • When appropriate, Dr. Greenstein will meet with family/powers of attorney prior to the in-person visit to learn more about the concerns.

  • Prior to the initial visit, Dr. Greenstein will review medical records and speak with members of your care team to learn more about your history and present needs. 

  • During the consultation, there will be a thorough psychiatric evaluation, which includes:

  • Learning about challenges in your life and completing aging-related assesments.

  • Reviewing medications, mental health history and medical history.

  • Completing mental status, cognitive and neurological exams when indicated.

Following the exam:

  • There may be a need to communicate with caregivers and other members of your care team as part of the treatment plan formulation.

  • A treatment plan will be communicated to you and your referring physician.

  • There may be recommendations for starting or stopping certain medications, additional work up (labs, brain imaging, neuropsychological testing), therapeutic interventions, etc.

  • The frequency of follow-up visits will be determined as part of the care plan.

  • Psychotherapy is integrated into all visits conducted with patients and their family members. Techniques utilized include supportive psychotherapy, problem solving therapy, cognitive behavioral therapy, motivational interviewing and psychoeducation. 

Follow up visits:

Follow-up visits are scheduled for 30 or 60 minutes. Face-to-face time is typically 25 minutes and 45 minutes, respectively. The rest of the time is spent on care coordination, communication and documentation.

Consultation and Advisory Services

Dr. Greenstein offers advisory and consulting services to clinical startups and health technology companies, healthcare organizations and investors who want to better understand the care delivery and payment landscape for older adult mental health and dementia. Dr. Greenstein's introduction to value-based care and industry began when he was a fellow in geriatric psychiatry. During that time, he served on a dementia workgroup for Mass General Brigham’s Department of Population Health Management. After completing fellowship, Dr. Greenstein was the founding chief medical officer of Rippl Care, a digital health platform for older adult mental health and dementia.

Dr. Greenstein’s unique perspective formed from experiences in academia, industry, government, and community-based practice allow him to provide services to an array of organizations seeking to learn about the market, review potential investments, launch a start-up or refine an existing product.

Legal and Forensic

Dr. Greenstein conducts Court-ordered and other requested evaluations in the State of Colorado for questions regarding decisional and functional capacity in aging people.
